Frequently Asked Questions

The Belmont Medical History and Immunization Record are only available to deposited students. Student Health Portal accounts will be activated within 1-2 weeks of their Admissions deposit. Upon accessing the Portal, if a message says "cannot authenticate log-in," please try again the following week. In some cases, access may be granted as late as 5 weeks following your Admissions deposit. Please notify Health Services if you are unable to access your Health Portal after 5 weeks have passed.

Also, if you created your myBelmont account within 24 to 48 hours of attempting to access the Health Portal, access may not be allowed. Please wait at least 48 hours after creating your myBelmont account before accessing your Health Portal. If you are unable to access the Health Portal following 48 hours, please contact Health Services.

Additionally, if neither of the above scenarios apply to you, you may need to clear your browser's cached data. Google "how to clear cache" for the easiest instructions on this process.

To allow time to process, please complete all health requirements by these dates:

June 1 (For students planning to enter in the Fall Semester)
Dec. 1 (For students planning to enter in the Spring Semester)
May 1 (For students planning to enter in the Summer Semester)

Yes, You must enter your vaccine dates first then the system will redirect you to upload the documentation. You must upload a document or the dates you entered WILL NOT save and you will have to repeat the process. A step-by-step guide is available on your health portal or by emailing us at healthservices@belmont.edu.

If you are using a Macbook to complete the requirements, please use Google Chrome, Firefox, or Microsoft Edge web browsers. The Safari web browser has been known to cause issues when completing the requirements. 

Please ensure that documents being uploaded to the health portal must be in an acceptable file format. The types of acceptable file types are:.gif, .png, .tiff, .tif, .jpg, .jpeg. Documents must be .txt or .pdf). If unsupported file types are uploaded the system will not accept the document. 

If you are trying to upload multiple documents. Please submit one document at a time. After submitting documentation with your immunization dates, additional documents can be submitted under the “uploads” tab in your health portal.

All health requirements must be completed online in the student's health portal which is accessed through their MyBBelmont account or directly from the health services home page. The easiest way to upload your documents is to take a picture of the document with your smartphone and upload it after logging in to your Belmont health portal. Immunization records can be obtained from your physician/pediatrician, your state health department, or previous institutions you have provided records to (high school/previous universities.) Only you will know who has copies of your immunization records. If you cannot find any history of previous immunizations, but are sure that you have received vaccines previously, positive blood tests for immunity may be submitted to complete the vaccine requirements. Please email or call our clinic for more information if you are unsure.

Immunization records may be faxed to health services. PLEASE BE AWARE, it is the student's responsibility to pick up their documentation from the clinic and complete the health requirements in their portal. Health services does NOT enter the dates and upload the documentation for them. To find out if your faxed records have been received you can email us at healthservice@belmont.edu.

Submitted documentation, dates, and forms must be manually reviewed by health services staff. Please give us 48-72 hours to review the items you submit. If your record has been reviewed and you are still not compliant, please check your messages within your health portal to see what requirements have not been met and email any further questions to healthservice@belmont.edu.

Yes, the state of Tennessee allows for medical or religious waiver of required immunizations. 

Our priority in Health Services is to keep all students, faculty and staff safe and healthy while on Belmont’s campus and in the surrounding Nashville community.

Health Services follows the recommendations from the CDC and the Tennessee Department of Health regarding vaccine administration for college students. The State of Tennessee requires all students attending college to be vaccinated against harmful diseases as college campuses are unique spaces for widespread disease transmission. If you have a medical or religious request to be exempt from certain vaccinations, please call 615.460.5506 and a nurse will explain this process.

Please visit the CDC and the Tennessee State Department of Health for more information on immunization requirements and recommendations.

Students who are online only and will not be present on Campus for the entire semester may submit an online only waiver for the vaccine requirements. Please note that the medical history form and consent for treatment and privacy practices must still be completed. Additionally, the waivers are only valid for one semester and will need to be completed again the following semester if you will remain online only.

Active service members or veterans attending Belmont University can submit their Military ID or DD214 to satisfy immunization requirements. Please note that the medical history form and student consent for treatment must still be completed for compliance. Also, be aware these documents may not satisfy individual program requirements, and you should contact your program coordinator if you are unsure.

Student athletes who submitted immunization records to the athletics department will still need to complete the university's health requirements. They will need to log into their health portal and complete all requirements including uploading the documentation and entering the dates for their immunizations.

The following immunizations are required for students planning to have patient contact for their program. (i.e. Nursing, Pharmacy, Medicine, OT, PT): 

  • Positive blood test for immunity for Hepatitis B, MMR, & Varicella 
  • Tetanus (TDaP) shot within the past 10 years 
  • TB skin test within the past year (regardless of screening question answers)
