Work-Related Injuries
Belmont University is required by the Department of Labor to record and report all work-related injuries. If you are injured performing your job duties, please follow the instructions below:
- Fill out a First Report of Injury Form.
- Choose a clinic from the Panel of Physicians or the Orthopaedic Physicians Panel.
- Notify the Office of Safety Programs (OSP) at 460-6023.
- If the OSP is unavailable, notify University Counsel at 460-6441.
In the event an injury is so severe that the employee is unable to follow the above steps, call Campus Security at 6911 to get an ambulance for the injured employee. After the injured employee is on the way to the hospital, his or her immediate supervisor should fill out the First Report of Injury form and notify Safety Programs or University Counsel.
