Immunization Requirements
(from the Pharmacy Student Manual)
All Belmont students must file a completed health form with:
- The Office of Student Health Services
- The Belmont University School of Pharmacy (BUSOP), Office of Experiential Education
- Retain a copy for your records
Required:
Side one of the health form:
- Personal Information
- Health insurance information (please be sure to attach a copy of the front and back of your insurance card)
Side two of the health form:
- Section A: Tetanus shot within the last 10 years
- Section B: MMR (Measles, Mumps, & Rubella) vaccination after 1989
- If you elect to have the titer drawn for MMR and the results are negative, you will be required to have an additional dose of the vaccination and have the titer drawn again
- Section C: Hepatits B
- Three doses of the vaccine are required for optimal protection. Missed doses can be completed if at least one dose has been obtained and proof can be provided.
- Section E: TB skin test (You must show proof of having receive the TWO-STEP TB skin test even though that is not specified on the form). For more information on the Two-Step TB skin test click here.
- Secton F: Varicella Titer (chicken pox)
- You must have a titer drawn. Having had the virus does not guarantee immunity. If you do not have a positive titer, you must receive an additional vaccination(s). Should this occur, you will also have to have a new titer drawn at the time period specified by your physician.
- Section G: Required only if vaccinations not complete or official documentation of vaccinations cannot be provided.
- Additional vaccines or testing may apply to specific Pharmacy Practice Sites. Instructions will be given on an individual basis in these instances.
It is very important that a student list any medical conditions/medications on his or her health form in case of an emergency situation. NOTE: students not submitting the health form will be restricted from course registration for the subsequent semester until the health form has been completed. Failure to have a COMPLETE health record on file in the Office of Experiential Education will result in an inability to take the Introductory Pharmacy Practice Experience I (IPPE I) course in the spring, which is mandatory for academic progression.
