Nursing Clinical Health Records Checklist


Health Records Deadline for Nursing Students Beginning Clinical Rotations in Spring 2010:  November 24, 4pm. 

Two – Step TB Test results by Mantoux:  Must be as recent as 3 months before the first day of your classes.  (*If you have tested positive, a chest x-ray or validation of treatment from a medical professional must be submitted.)  Please Note:  This is NOT a traditional TB test.  Click here for an explanation of the two step TB test for your health care professionals reference if needed.

Proof/date of a Tetanus within the past 10 years

Two Measles, Mumps and Rubella (MMR) Vaccine doses/dates

Three Hepatitis B Vaccine doses/dates (Verification that you have started the vaccine series will be sufficient to begin clinical if you have not had time to sufficiently complete the series.  You will be expected to continue on schedule with the series while enrolled in courses.) 

Flu Vaccination: Documentation that you have received an annual flu vaccination from fall of 2008 (This requirement is seasonal and students beginning clinical rotations in the fall semester are exempted for the time being)

OFFICIAL LAB REPORT FOR ALL  FIVE OF THE FOLLOWING TITERS REQUIRED 

Titer lab work is generally run between 1-2 months after the completion of the entire series, it may be run later than that but no sooner than four weeks from completion.

  • Hepatitis B Titer
  • Varicella IgG Titer
  • Measles (Rubeola) IgG Titer
  • Mumps IgG Titer
  • Rubella IgG Titer

 A photo copy of a current valid CPR card (front and back).  Only two courses fulfill the requirement.  For a list of agencies that provide adequate training click here
