Once you have received your letter of acceptance from Belmont, paid your $3000 enrollment deposit and returned your notarized copy of the International Student Sponsor's statement, please complete the top portion of the SEVIS Transfer Request Form. Submit it to the international student adviser at your current school. Ask them to complete the lower portion and fax or mail the form to us at the fax number or address on the form.
DO NOT, UNDER ANY CIRCUMSTANCES, complete this form until you have
- Received your letter of acceptance from Belmont
- Paid your $3000 enrollment deposit
- Returned your notarized copy of the International Student Sponsor's statement
We cannot create a new Belmont I-20 for you, even if your current school has transfered your SEVIS record to Belmont, until the above steps have been completed.
