How to Complete

Completing the Belmont Medical History and Immunization Records

The confidential Belmont Medical History Form and Immunization Record are housed within the Health Services’ secure online Portal in myBelmont. This process works best with Firefox or Chrome.

Steps to completion:

  1. Log-in to your myBelmont account.
  2. Click on “Health Services" located on the left side of your screen, then "Health Portal" (or click the hyperlink here).
  3. Carefully read instructions, and then click on the "Go to Health Portal" button.
  4. At the top toolbar, select "Forms," then "Belmont Medical History."  Answer each question and submit. You must answer EVERY question in order to successfully submit the form.
  5. Then select "Immunizations" on the toolbar and "Required for All Students"; enter dates from your own immunization/vaccination records, then press “Submit.” You may also enter other immunizations and lab tests for immunity. Simply select & enter dates under the appropriate category (Recommended, Travel, etc.)
  6. Go to "Upload" on the toolbar and upload a copy of your immunization records and a front/back copy of your insurance card.
  7. Additionally, please complete the "Student Consent for Treatment and Privacy Practices" by reading the document and 1) signing the electronic signature option in "Forms" OR 2) printing, signing, and uploading an image of the signed portion in "Uploads." If you are under 18 years of age, you must print the signature page and have it co-signed by a parent or guardian. This a new requirement beginning June 2019.
  • Tip: The easiest way to complete all uploads is with a mobile device! First, use your mobile device to take pictures of the documents you are uploading then log-in to your myBelmont and Health Portal using your device’s web browser
    • Next, scroll down to “To Do List” and select “Uploads”
    • Choose the document type you are uploading from the drop-down menu
    • Click “Select File” then click to access your mobile device’s image/photo/document library
    • Select the image you wish to upload
    • Finalize by tapping the “Upload” button that appears.
  1. Your immunizaton records and Health Form are hand checked and a message will be sent to your Health Portal if you are missing any requirements. To review messages, please select "Messages" in your Health Portal.

IMPORTANT: All documentation (immunization record, health insurance, and "Student Consent for Treatment and Privacy Practices"), forms (Belmont Medical History), and processes (immunization dates entered into your Health Portal) must be completed and verified to avoid a Registration Hold.

NOTE: If your records are faxed to Belmont University Health Services, you are required to retrieve them and complete all steps associated with your Health Requirements.

Cannot Access the Form?

The Belmont Medical History and Immunization Record are only available to deposited students.  Student Health Portal accounts will be activated within 1-2 weeks of their Admissions deposit.  Upon accessing the Portal, if a message says, "cannot authenticate log-in," please try again the following week.  In some cases, access may be granted as late as 5 weeks following your Admissions deposit.  Please notify Health Services if you are unable to access your Health Portal after 5 weeks have passed.

Also, if you created your myBelmont account within 24 to 48 hours of attempting to access the Health Portal, access may not be allowed.  Please wait at least 48 hours after creating your myBelmont account before accessing your Health Portal.  If you are unable to access the Health Portal following 48 hours, please contact Health Services.

Additionally, if neither of the above scenarios apply to you, you may need to clear your browser's cached data. Google "how to clear cache" for the easiest instructions on this process.

Paperwork and Documentation Help 

Immunization records:  Students will need their childhood immunization records to complete the immunization requirement.  If students no longer have copies of their immunization records, the records may possibly be obtained from students' primary provider, health department, high school, or previous university.

If records cannot be obtained, blood work (titers) may be drawn at Health Services to test for immunity to Hepatitis B, Measles, Mumps, Rubella, and Varicella. Currently, we are unable to test for Meningitis and Tetanus. Please call us at 615-460-5506 for pricing on titers.

If you need any of the required immunizations, you may receive them from Health Services, as well. Please call 615-460-5506 for pricing on immunizations.

Current Requirements for All Students Include:

  • Tetanus or Tdap booster within 10 years of the first day of class.
  • Two Measles, Mumps, & Rubella (MMR) vaccinations for those born after 1956, or positive titers.
  • Three doses of Hepatitis B, or positive titer.
  • At least one dose of Meningococcal vaccine (effective against types A, C, W, and Y) given at 16 years of age or later for students living in on-campus housing.
  • Two doses of Varicella (chickenpox) vaccine, or positive titer, or medical documentation with date of chickenpox disease for those born after December 31, 1979.
  • TB skin test only if you answered “yes” to a TB Question on the Belmont Medical History.

Current health insurance coverage:  Please upload a front/back copy of your insurance card into the Health Portal.

Health Services does not charge an office fee for visits; however, some costs may be incurred for immunizations, lab testing, etc. Immunization pricing is available by telephone at 615-460-5506. Health Services does not bill insurance, but will accept cash, check, credit, or debit for payment. If you wish, we can provide an itemized statement that you may file with insurance; this same document may be printed from your Health Portal under "Statements."
