School of Nursing Clinical Record Requirements
Health record requirements are mandatory for all nursing students and must be completed in order to participate in clinical rotations in compliance with hospital requirements. Student's timely cooperation in submitting the following information is expected. Late or incomplete records will result in an inability to participate in the clinical rotations, which are a degree completion requirement.
A Belmont University Student Health Form was included in your acceptance package. Please Note: The School of Nursing and Student Health Services are two separate departments and the School of Nursing requirements are broader than the Belmont University Health Form; it is the student's responsibility to ensure that both departments receive required information respectively. (Partnership students enrolled in Trevecca Nazarene University are expected to follow the requirements of both the SON and SHS.) It is the student's responsibility to ensure each department receives the required information.
Some requirements overlap, so we recommend you make copies of your Student Health Form and submit copies to the School of Nursing directly, along with the additional items required by the School Of Nursing. If you have already submitted your form to Student Health Services and did not keep a copy, please contact them about obtaining a copy. Do not neglect to send your original health form to Belmont University Student Health Services at 1900 Belmont Blvd, Nashville TN 37212 or by fax, (615) 460-6131. Failure to submit this to Student Health Services may result in a hold on your student account in the future.
SON Health Records Deadline:
The deadline for submitting health record information to the Belmont University School of Nursing for participation in Spring 2010 clinical rotations is November 24th, 4pm CST.
Follow the Nursing Clinical Health Records Checklist carefully when compiling your information (except where noted, all health records, forms and card copies must be submitted in their entirety, and will not be accepted piece by piece.)
The School of Nursing does not accept electronic health information via email, instead you may submit it by:
- Dropping it by the office in person, suite 106, 1st floor IHSB
- Faxing it to 615-460-5644

Money Saving Tip: A full range of immunization and vaccination services may be obtained, by appointment, through Belmont University Student Health Services by calling 615-460-5506. Most health care insurance providers will not cover the cost of adult immunizations. While some of the standard immunization fees and services offered through Student Health Services are on par with the average provider, many of their services save students as much as 50-80%. The majority of private health care providers outsource titer lab work to a third party, at a much higher price than the rates charged at Student Health saving our students, on average hundreds of dollars.
